Seniors

Homemaker Services program: provides disabled/low-income seniors routine housekeeping services.

75% clients in home annually. 

The Senior Resources Program has experienced a decrease in hours due to the death of elderly clients. Because of this, the program supervisor is doing many intakes to boost the number of clients. In the meantime, the two part-time homemakers hired in Q4 2024 are helping with the chore program to reduce the waitlist.

In Q4 2024, 98.66% of clients were able to remain in their home. This is above the target of 75% giving this component a green status and providing 1.0 point to the overall Vulnerable Populations measure.

Veterans

Veterans Services Office –Assists Arapahoe County veterans and their dependents in obtaining eligible VA benefits, state veteran benefits, and appropriate community resources. 

1,500 veterans/families receiving secondary contacts from the Veterans Service Office. 

In Q4 2024, the A/D Works! Work Based Learning (WBL) Program provided administrative support to the Veteran Services Office (VSO). This was necessary given the Healing Warriors Program is located in Aurora while the WBL is located in Littleton along with the VSO. 

The VSO is also working with the Colorado Department of Health and Human Services (CDHHS) to share a staff member to assist with processing rent assistance for veterans. This help is essential in meeting our goals moving forward.

Judicial Offenders

Pretrial Services Navigator program – assists individuals with behavioral health disorders currently involved with the Arapahoe County Courts navigate the justice and public benefits systems.

80% successful termination rate. 

In Q4 2024, there were 17 successful terminations and 5 unsuccessful terminations which is a success rate of 77.25%. While this is below the target of 80%, it is within the acceptable range and therefore receives a green status. This component will contribute 1.0 point to the overall Vulnerable Populations measure.

Adult Protection Services

This measure gauges the ability of the Arapahoe County to make state-required monthly contact with clients who have open cases.

At least 95% of monthly contacts are made within the state-required timeframe of 35 days.

Please note this measure component is reported 1 quarter in arears. In Q3 2024, APS met Timely Contacts in all three months. ACDHS reviews this measure monthly and examines cases where there is a late contact.

This component will contribute 1.0 point to the overall Vulnerable Populations measure.

Inmates

Arapahoe County Sheriff’s Office - provides mental health services to current inmates within the Arapahoe County detention system.

In Q4 2024, performance remained high for this measure.. The Mental Health team contacted 5,098 patients per month throughout the quarter. We continue to enjoy having a full team of Mental Health Professionals which certainly correlates to a better and more efficient delivery of services.

This can likely be associated to increased availability by the Licensed Mental Health Professionals, but also due to increased acuity displayed by the inmate-patient population. The delivery of quality mental health services is a priority to the Arapahoe County Detention Facility, and we will continue to evaluate best practices that will enhance the delivery of services to this high-risk patient population.

While the use of tele-psychiatry is still an option, we do have two psychiatry providers that manage medications. We continue to offer group therapy to the individuals experiencing incarceration at the Arapahoe County Detention Facility. We are hoping that this effort will prove beneficial as the goal of the group therapy is to focus on enhancing coping mechanisms which will lead to a healthier incarceration but will also assist in preparing for adversity post-release. The group therapy is being offered by our behavioral health manager and a community partner at least once per week, but sometimes twice per week.
